I discovered it when I activated a bridge too soon and the whole bridge went up and my car went into the water. I swam between the docks and a ship and they lost me even though I was completely surrounded. I might try to make a video of this later.

I disabled a helicopter twice in a mission in Act 1 and I wondered why it disappeared. I thought that maybe I took out everyone except the helicopter, but then it was gone.

I'll have to keep the water escape in mind.. another good tip.

I'm glad I accidentally figured it out, because one of the later missions is a bitch. I failed 3-5 times because I always got a shit car or bike per usual, so I eventually Solid Snaked it into the water and swam away. The undetection rate was infinity so hiding out of sight wasn't an option. I swam for 20 minutes to get out of the circle and escape.
